Campaign Statements - Forms for Filing
The Act requires candidates and committees to file campaign statements and reports so that voters are fully informed about contributions received and expenditures made in elections. Candidates and committees use campaign forms to report this activity. Filers should always check with their filing officer to determine if certain filing methods are required in their jurisdiction.

Major Donor & Independent Expenditure Committees not receiving contributions from other sources file:

- Form 462 - Verification of Independent Expenditures
- Form 496 - 24-hour/10-day Independent Expenditure Report

- Form 497 - 24-hour/10-day Contribution Report
Slate Mailer Organizations involved in the production of one or more slate mailers and that exercise control over the selection of candidates and measures to be supported or opposed in the slate mailers; and receives or is promised payments totaling $500 or more in a calendar year for the production of one or more slate mailers file:
- Form T10 - Committees Primarily Formed for State Ballot Measures or Candidates
